Thank you for choosing The William W. Backus Hospital. We hope to help guide you through your experience, and answer any questions you might have. The hospital is a large and sometimes complex place, and this handbook contains a wide range of information from
pre-registration to discharge to help improve your experience at Backus.

Of course we realize no publication or website can take the place of one-on-one interaction, and our staff prides itself on being able to support you and your family during your stay. Please don’t hesitate to ask questions of our staff members; we strive to put our patients first – always.


Medical Records

The William W. Backus Hospital’s Medical Records Health Information Department is the administrative office of the hospital responsible for ensuring that patients’ medical records are complete, accurate, confidential and readily available for future patient care and clinical communication.

Copies of medical records/health information can only be released with the permission of the patient. Patients treated at The William W. Backus Hospital can request a copy of their medical records by submitting a signed Authorization to Disclose Health Information.

If the patient is currently less than 18 years old, his/her parent or guardian must sign the Authorization to Disclose Health Information.

Include your full name, date of birth, current address, phone number, dates of service here at The William W. Backus Hospital and specify what information you want sent from your medical record. Please be as specific as possible. The completed form can be returned to the Medical Record Department/Health Information Department either by fax, email, or general postal mail.

There is a fee of 65 cents per page for medical and/or patient records that are to be sent directly to you. If copies are sent directly to a physician or hospital, there is no charge. When completing the request form please print the mailing address in which you would like to have the data sent to. When picking up medical records please bring photo identification, such as a driver’s license.

Private Health Information

To inquire as to when private health information will be ready for pick or if it has been sent to requesting providers, please email Medical Records.

Psychiatric Records

Some restrictions may apply. Please call Medical Records before completing the request and authorization forms.

Social Security Numbers for Newborns

Please contact the Social Security Department in Willimantic, CT to inquire about social security numbers for newborns


Cell phones

Cell phone use is limited to designated areas only.

The designated areas include the main lobby, the cafeteria, the Emergency Department waiting room, the Hospital Annex and any outside location on the Hospital campus. Otherwise, cell phones should be completely turned off.


Pastoral care

The Hospital’s Pastoral Care Department, staffed by Roman Catholic and Protestant chaplains, is available to assist all patients and their families with spiritual needs.

If you would like to see a Chaplain, ask a member of the nursing staff or the hospital operator to call beeper #0220. The Hospital also maintains a chapel open to the public on the ground floor.
