The Hartford HealthCare Acute Spine Program specializes in treating adults struggling with cervical or lumbar pain, including:
- New onset neck
- Low back pain
- Radicular pain less than four weeks in duration
You’ll benefit from our team approach, which includes partnerships with radiology, physical therapy, integrative medicine, neurosurgeons and behavioral health specialists. Your team of specialists will work closely so you can be sure you’ll receive comprehensive care for your spine condition.
Treatment plans focus on decreasing your pain and optimizing your functional capabilities through prescribed exercise, hands-on care, and patient education. Your providers will work with you on developing an individualized treatment program.

Your nurse navigator
As part of the program, our nurse navigator will help coordinate your care. Your navigator will assist you in coordinating your recommended care as well as following you throughout the duration of your care within the program to ensure you are progressing.
At the time of initial contact, the nurse navigator will complete an extensive medical and health history. The information we gather at each visit helps us develop an individualized, interdisciplinary care plan that aims to get you back on your feet faster!
How to make an appointment
To obtain a referral, discuss the program with your physician and ask for a referral or call the nurse navigator directly. Self-referrals are welcomed.
Once the nurse navigator receives the referral, you will receive a call within 24-48 hours to establish an appointment with a provider. Appointments will be provided within 24-48 hours of initial contact.
